The isoxazoline four brothers: a chasm in adverse-event reporting volume
Four veterinary ectoparasiticides share the same chemical scaffold and the same mode of action. Afoxolaner (้ฟ็ฆๆ็บณ), fluralaner (ๆฐ้ทๆ็บณ), sarolaner (ๆฒ็ฝๆ็บณ) and lotilaner (ๆดๆฟๆ็บณ) are all isoxazoline-class agents โ they block insect and acarine GABA- and glutamate-gated chloride channels, killing fleas and ticks. In the United States they are all captured in the FDA Center for Veterinary Medicine's adverse-event database, exposed through the openFDA animalandveterinary/event.json endpoint. They are, in a very real sense, four brothers from one chemical family.
And yet when you line up their adverse-event (ADE) report counts, the four brothers are not standing shoulder to shoulder. Two of them carry report volumes near or above 80,000; one sits just under 70,000; and the youngest trails by an order of magnitude. This article is the sister piece to the fluralaner deep-dive โ here we widen the lens to all four and ask a deceptively simple question: why does the same class show such a chasm in reporting volume, when their market footprint is, on paper, far more even?
The headline numbers
All figures below are cumulative FDA animal-and-veterinary adverse-event report totals, pulled from openFDA at the snapshot dated 2026-07-21 / 2026-07-22 (the source topics were pulled on both dates and carry identical totals). The four brothers, ranked by total reports:
- Fluralaner (ๆฐ้ทๆ็บณ): 88486
- Sarolaner (ๆฒ็ฝๆ็บณ): 79692
- Afoxolaner (้ฟ็ฆๆ็บณ): 69037
- Lotilaner (ๆดๆฟๆ็บณ): 13432
Read plainly: fluralaner and sarolaner occupy the 80kโ89k band; afoxolaner is a close third at just under 70k; and lotilaner sits far below at roughly 13k โ about one-fifth of afoxolaner's total and one-seventh of fluralaner's.
Critical disclaimer โ read before interpreting any number above. Every count in this article is cumulative as of the openFDA pull snapshot 2026-07-21 / 2026-07-22. A report being filed in the FDA CVM database does not establish that the drug caused the event: reported โ causal. These are spontaneous, passive-surveillance reports submitted by veterinarians, pet owners, and manufacturers; they are not controlled studies and carry no causal attribution. Furthermore, under the VEDDRA coding scheme used here, "Lack of efficacy" terms denote product-performance reports (the product didn't kill the parasite), not adverse reactions in the animal. They are reported separately and must not be read as toxicity.
A chasm against market breadth
The obvious hypothesis is that report volume tracks market size: the more animals treated, the more reports filed. If that were the whole story, lotilaner's low count would simply mean a smaller market. But the available market-authorization data complicates the tidy version of that story.
Species breadth โ where the reports actually land
The dog counts show all four brothers are overwhelmingly canine products, but with different shapes:
- Fluralaner: Dog 65708 of 88486
- Sarolaner: Dog 67742 of 79692
- Afoxolaner: Dog 63981 of 69037
- Lotilaner: Dog 11534 of 13432
Now look at the cat signal โ the clearest breadth marker in the set:
- Fluralaner: Cat 13343
- Sarolaner: Cat 8207
- Lotilaner: Cat 859
- Afoxolaner: Cat 82
Fluralaner's 13343 cat reports are roughly 15% of its entire total โ a substantial multi-species companion-animal footprint. Sarolaner, too, shows meaningful cat exposure (8207). By contrast, afoxolaner reports are almost purely canine (82 cat of 69037), and lotilaner's cat count (859) is a small slice of a small total.
Human exposure reports widen the picture further: fluralaner carries 1288 human reports, sarolaner 143, afoxolaner 92, lotilaner 27. Fluralaner additionally records Rabbit 6, Fox 3, Goat 3; sarolaner Rabbit 3, Bobcat 2, Lynx 2; afoxolaner Other 3, Other Canids 3; lotilaner Cattle 1, Pig 1. The breadth of species touched is itself uneven across the brothers.
Market-authorization breadth on the record
From the approval-label topics available in the source set, two of the four brothers have a first-hand Canadian market-authorization record on file:

A note of honesty: approval-label topics for fluralaner and lotilaner products were not present in the source material used here. So any breadth claim about those two rests on the ADE species signal above, not on a registration record in this dataset. We do not fill the gap with assumption.
What's actually being reported
Stripping out the "lack of efficacy" product-performance reports, the clinical reaction profile is dominated by gastrointestinal and neurological terms across all four. Verbatim top VEDDRA terms from the source topics:

Two things stand out. First, Seizure NOS appears in every brother: 4406 (afoxolaner), 3347 (fluralaner), 7249 (sarolaner), 729 (lotilaner). The neurological signal is class-wide, not unique to one molecule. Second, sarolaner's top clinical reaction is Seizure NOS (7249) โ the highest seizure count of the four โ while fluralaner's headline is lifted by a massive Lack of efficacy (ectoparasite) - flea 17108, a product-performance term that is not an adverse reaction.
The four in the dog landscape
Zooming out to the whole canine adverse-event universe, the ade_top_actives_dog topic reports 985705 total Dog ADE reports. The isoxazolines appear inside the upper ranks of that list:

For scale, the very top of the dog list is Spinosad 180156, Milbemycin Oxime 168422, Ivermectin 103618, Pyrantel As Pamoate Salt 82225, Moxidectin 59458. Fluralaner is split across two distinct product-name entries (Chew Tablets and the 13.64% 12-Week Chew), which is itself a reminder that the ingredient-level totals in the first section are not directly comparable to the product-level rows here โ combo and branded forms fragment the same molecule.
Why the chasm, then?
The data on disk does not let us prove a cause for the volume gap โ and per the disclaimer, it was never designed to. But it does let us reject the laziest explanation. Report volume is not a clean proxy for either intrinsic danger or for simple market share: lotilaner's low total coincides with a narrow species footprint (mostly dogs, 11534 of 13432, with only 859 cat and 27 human), while fluralaner's high total rides on the broadest species spread of the four (13343 cat, 1288 human, plus rabbit/fox/goat). Sarolaner's 79692 is buoyed by strong dog and cat numbers and a notably high seizure count. Afoxolaner, though third in volume, is almost purely canine in its reporting.
In other words, the "chasm" is partly a chasm of exposure surface โ how many species, how many markets, how many combination products โ rather than a chasm of per-animal risk. That is precisely the kind of nuance a raw ranking hides, and the reason this series treats each molecule on its own terms before drawing any cross-class conclusion.
