# When food is the medicine: mapping feline diet–disease evidence clusters

Cats are obligate carnivores with a narrow physiological tolerance for dietary error — a fact that makes "diet as therapy" unusually central to feline medicine. For at least six common cat conditions, a therapeutic diet is not a supplement to treatment but the treatment: restricted-iodine food for hyperthyroidism, phosphate-restricted food for chronic kidney disease, dissolution diets for urinary stones, high-protein/low-carbohydrate food for diabetes. Yet "there is a diet for that" and "the literature actually supports that diet" are two different claims. This article maps seven feline diet–disease areas against their Europe PMC evidence clusters and asks a blunt question: how much of the volume is on-target, and how much is noise?

The data behind this map comes from first-hand Europe PMC (Europe PubMed Central) REST searches pulled in July 2026, each returning a hit count and a handful of representative peer-reviewed abstracts. The hit count is a proxy for *literature volume*, not *diet-specific evidence quality* — a distinction that matters below.

The pattern is inverted: the largest clusters are not the best-supported diet interventions.

## Obesity — large volume, mostly about risk

At 28,780 records, feline obesity is by far the largest cluster, which fits the clinical reality. As one 2026 Frontiers in Veterinary Science review puts it plainly, "Overweight and obesity represent the most common nutritional disorder in domestic cats." The cluster also surfaces useful risk-factor work — for example, a 2026 study finding "Feline obesity is associated with stronger owner attachment, while indoor confinement increases risk of obesity at an early age in domestic shorthaired cats" — and mechanistic work such as a Journal of Nutrition serum-metabolomics study comparing low-protein, low-fat and low-carbohydrate diets. What the raw count *hides* is that most of this literature is epidemiological or metabolic, not a test of a specific weight-loss diet. The volume is real; the diet-intervention evidence is thinner than 28,780 suggests.

## Gastrointestinal — the strongest on-target signal

The GI cluster (2,561 hits) is the one place where a genuine controlled diet trial surfaces near the top. A 2026 Journal of Feline Medicine and Surgery paper is titled "A psyllium-supplemented gastrointestinal diet is effective for the management of chronic constipation in cats: a 6-month controlled clinical trial" — a multicentre, controlled, blinded trial, exactly the design tier the noisier clusters lack. The same cluster includes "The Effects of Dietary Enterococcus faecalis HHP003 Supplementation on Gut Microbiota Composition and Gut Health in Cats with Mild Diarrhea." For a cat owner asking "will a specific diet help my cat's gut," this cluster answers more directly than any other on the map.

## Chronic kidney disease — prevalence is well documented, diet mechanism less so

CKD is the condition where the *burden* is best quantified. A 2026 Veterinary Sciences paper states "Feline chronic kidney disease (fCKD) is highly prevalent in geriatric cats, affecting up to 80%," and a companion paper calls it "a leading cause of mortality in geriatric cats." A Portugal caregiver-perspective study (PMID 41398709) and a Greece real-world data analysis round out a solid epidemiological base. But notice the caveat: the CKD topic's very first representative entry is explicitly "Off-topic… lower-urinary-tract obstruction, not chronic kidney disease diet — mis-clustered into this topic." The phosphate-restriction diet that defines CKD management is assumed by the literature more than it is *tested* in the surfaced abstracts.

## Hyperthyroidism — the cluster is noisy

With 1,550 hits, hyperthyroidism looks modest, but the representative set is revealingly off-center. Only one of six entries — "Hyperthyroidism is a common endocrine disorder in elderly cats, often leading to concurrent cardiac abnormalities" — is squarely on topic. The others are renal RAAS markers, tongue infarction in a cat with hypertrophic cardiomyopathy, epileptic seizures, and portal vein hypoplasia. The restricted-iodine diet that is the cornerstone of feline hyperthyroidism management is the clinical backdrop, not the surfaced evidence. So the cluster *exists*; the diet-specific signal is buried.

## Lower urinary tract disease — obstruction outcomes, not dissolution

The 1,283-hit LUTD cluster returns a 2026 multicentre retrospective study of "601 male cats (2020–2024)" comparing azotaemia and time to resolution between initial and recurrent urethral obstruction, and a Chinese pet-insurance survival analysis of FLUTD described as "a common multifactorial syndrome." Useful for understanding obstruction — but the therapeutic-diet question (struvite dissolution, urine-acidification, saturation control) is not what these abstracts answer. The cluster measures the *problem* more than the *diet fix*.

## Diabetes — guidance plus sobering outcomes

Diabetes (1,227 hits) is the most balanced cluster on the map. It surfaces a proposed framework — "Guidelines for nutritional management of feline diabetes mellitus: a proposed classification system integrating medical considerations" — and a hard outcome statistic: "Approximately 10% of cats with diabetes mellitus are euthanased at diagnosis and a further 10% euthanased within the first year of treatment." That pairing — a nutritional-classification effort plus candid mortality data — is closer to actionable evidence than the volume-led clusters above.

## Dental — the weakest cat-specific signal

The dental cluster reports 3,747 records, but almost all representative entries are human dentistry: nanozyme engineering, clear-aligner surveys, protein restriction during odontogenesis, children's fluoride exposure. Only a functional-pet-food review ("Advances in Functional Pet Food Research: Health-Driven Ingredients, Nutritional Targets and Evidence-Based Claims") is pet-relevant, and it covers pet food broadly, not feline dental diets. For a cat owner weighing a "dental" kibble claim, this cluster offers almost no on-target support.

## The through-line: cats are obligate carnivores

Every one of these diet–disease links rests on a single biological premise. As the Merck Veterinary Manual summary in `companion_cat_nutrition` frames it, "Cats are obligate carnivores, eating chiefly meat," they "need high-quality fat and protein plus specific amino acids (e.g., taurine) absent from dog or human food," and an adequacy statement "confirms the food is complete and balanced for a life stage." Merck also notes "Wet or canned food boosts water intake — a common reason vets favor it over dry," a point that bears directly on the urinary and kidney clusters above. This is the reference backbone; the Europe PMC clusters are where the condition-specific claims are either sharpened or, as often, left untested.

## What the map actually shows

Ranked by *diet-specific* support rather than raw volume, the honest order is: GI (controlled trial) and diabetes (classification + outcomes) lead; CKD and LUTD sit in the middle (strong on burden, weaker on diet mechanism); hyperthyroidism and dental trail, with their clusters dominated by adjacent or human topics. Obesity is the paradox — vast literature, mostly about the problem rather than the diet.

The takeaway for anyone reading a "science-backed" feline diet claim: check whether the backing is a hit count or an on-target study. In this map, those are not the same thing.
